ABSTRACT

Objective To investigate the characteristics of distortion products otoacoustic emission (DPOAE) in hepatitis B carriers and chronic hepatitis B patients with normal pure tone results .Methods DPOAE was detected in 29 hepatitis B carriers (58 ears) ,30 chronic hepatitis B patients (60 ears)and 30 normal controls .The three groups had normal pure tone results .The detection rate and amplitude difference of DPOAE at 553~6250 Hz were compared between the two groups .Results The incidence of the DPOAE in HBV carriers for all the frequencies was higher than that in the chronic hepatitis B group ,but there was no significant difference (P>0 .05) .The level of the DPOAE in HBV carriers for all the frequencies was higher than that in chronic hepatitis B group ,and there were significant differences at 1105 ,1560 ,2211 ,3125 ,4416 ,6250 Hz (P<0 .05) ,and at 1560 ,2211 ,6250 Hz(P<0 .01) ,except at 553 ,783 Hz (P>0 .05) .Conclusion There is a potential loss of cochlear functions in the chronic hepatitis B group compared with the HBV carrying group .We suspect that the progress of liver disease is related to the potential hearing loss .

ABSTRACT

Objective To investigate the clinical application of Fibroscan to noninvasive detection of liver fibrosis in asymp‐tomatic carriers(ASCs)with chronic hepatitis B virus(HBV).Methods Clinical data of 195 chronic HBV ASCs were retrospec‐tively analyzed ,including liver function ,HBV DNA quantitation ,liver stiffness(LS) ,color Doppler ultrasound results of the liver and spleen and histopathological findings of the liver.LS was evaluated by transient elastography(Fibroscan).Liver biopsy was performed in 80 carriers for histological examination.Results The LS value was(6.22 ± 2.50)kPa in 195 cases.There was no significant difference in the LS value between HBV DNA 15 years was significantly higher than that in patients with HBV carrying dura‐tion≤5 years(P<0.01).It was (6.72 ± 2.33) kPa in patients(n=49)with liver tissue inflammation and necrosis

ABSTRACT

Objective To investigate clinical significance of the changes in the peripheral blood T lymphocytes subsets in patients with diseases related to HBV infection .Methods 257 cases of inpatients and outpatients were selected from Jan .to Dec .2013 ,and were divided into hepatitis B carriers (ASC)group ,chronic hepatitis B(CHB)group ,hepatocirrhosis(LC)group and primary liver cancer(PHC)group according to types of diseases related to HBV infection .Other 50 healthy individuals conducted physical exami‐nation were enro1led in the control group .The absolute CD3+ ,CD4+ and CD8+ T‐cell count ,and CD3+ ,CD4+ and CD8+ percent‐age and CD4+ /CD8+ value were detected in all subjects by using flow cytometer .These data were compared and analyzed .Results Compared with the control group ,there were no significant differences of the absolute CD3+ ,CD4+ and CD8+ T‐cell count ,and CD3+ ,CD4+ and CD8+ percentage and CD4+ /CD8+ value in ASC group ,CHB group and LC group(P>0 .05) .Compared with the control group ,the absolute CD4+ T‐cell count ,CD4+ percentage and the CD4+ /CD8+ value were decreased in the PHC group , while the CD8+ percentage were increased in the PHC group ,there were statistical significant differences between the two groups (P<0 .05) .Conclusion The peripheral blood T lymphocyte subsets could be monitor indexes of cell immune function in diseases related to HBV infection .

ABSTRACT

Objective To investigate the histological injury as well as the risk factors for the course of HBeAg-negative chronic hep-atitis B virus( HBV)-infected patients with persistently normal alanine aminotransferase ( ALT) levels ( PNAL) . Methods Ninty four patients of HBeAg-negative chronic HBV-infected with PNAL were performed liver puncturing. The ALT, HBV markers, and liver histo-logical changes were detected. The gender, age, family history, HBV-DNA and HBsAg levels were compared between different patients with liver inflammation and fibrosis. And the Logistic model was used during the analysis of independent risk factor. Results Among 94 cases, patients with inflammatory necrosis classification≥G2 accounted for 74. 5%, fibrosis grading≥S2 accounted for 27. 7%. Patients in the group of inflammatory necrosis classification≥G2 was older than the group of < G2, and the ALT level of≥G2 group was higher, as well as a higher HBV-DNA. What is more,Patients in the group of inflammatory necrosis classification ≥G2 were mainly male, and were associated with a family history of hepatitis B. Patients in the group of fibrosis stage ≥S2 was older than < S2 group, and the ALT level of fibrosis stage ≥S2 group was higher. And those patients were often associated with a family history of hepatitis B, but there was no significant difference among the HBV-DNA level and gender constitution. HBV-DNA and family history were the independent risk fac-tors of liver inflammation necrosis≥G2, and family history was the only independent risk factor of fibrosis stage≥S2. Conclusion The patients of HBeAg-negative chronic HBV-infected with PNAL, regardless of the ALT and HBV-DNA, must have a liver biopsy. The pa-tients with high level of HBV-DNA and with a family history of hepatitis B should be performed liver puncturing accordingly.

ABSTRACT

This study aimed to detect the prevalence of HBV carriers in pregnant woman by two blood tests, 3-month interval for HBsAg. At Siriraj antenatal clinic, there were 6.3% HBsAg positive and 37.6% HBsAg positive in 17,652 pregnant women who first visit ANC. Second blood test showed that 17.6% was HBsAg negative. HBV carrier pregnant women who were HBsAg positive twice were younger than HBV carriers who were HBsAg negative twice with P=0.00006. There were 1.2% high SGPT in HBV carriers. In sub-group who were HBsAg negative and turn to positive, 9.1% of them had high SGPT. This result indicates that in high prevalence of HBV carrier pregnant women blood test for SGPT in this sub-group is beneficial and cost-effective.
